Initial situation:
60 mln EUR was invested - but no software was ever developed. The IT project "Robaso" was stopped after 6 years of development.
Problem cause:
Software was developed under laboratory conditions. It was not tested in practice until 5 years after the start of the project. The complexity and possible sources of error were completely underestimated.
Loss:
60.000.000 €
Initial situation:
The Therac-25 radiation accelerator exposed patients with radiation levels 1000 times higher than normal.
Problem cause:
Defective software and complete lack of testing. Four bugs that led to over-irradiation were found in Therac-25's software. In addition, many potentially dangerous errors were discovered even before devastating consequences could result.
Loss:
From 1985 to 1987, Therac-25 caused six radiation overdoses. At least two patients died directly as a result of an overdose.
Initial situation:
The Golf 8 delivered in 2019 was struggling with software problems. It did not brake satisfactorily despite automated distance control and did not accurately recognize unusual road signs (speed 40 sign).
Problem cause:
Incorrect data detection/evaluation and malfunctioning manufacturer's factory software.
Loss:
Around 222,000 recalls, necessary software updates (€60 per update means €13,320,000 in workshop costs), and risk of distance control and cruise control failing to meet requirements, thus threatening the safety of passengers.
